Consent

Reframe Careers follows Australian privacy laws, including the Privacy Act 1998 and the NSW Health Records and Information Privacy Act 2002.  

Your personal information is collected only to provide you with vocational counselling services. We keep brief, relevant session records to support safe and effective care. 

Your information will not be shared with anyone outside Reframe Careers without your written consent. All information is stored securely and handled in line with our Privacy Policy: available on our website - click here to access

Your counsellor may consult with supervisors or colleagues for professional support. These people are also required to keep your information confidential.
 


With your permission, we may collaborate with other professionals involved in your supports. Only relevant information will be shared, we will confirm with you before contact is made, and you can withdraw your consent at any time.
 

We will not share your information without your consent unless required by law.

Disclosure of Personal Information 

Your personal information is kept confidential. It will only be shared in limited circumstances, including when: 

  • We are required to disclose information by law or court order 

  • There is a serious and immediate risk to your life, health, or safety, or to another person 

  • You have given your consent for us to: 

    • provide a written report to another professional (such as a psychologist), or 

    • discuss relevant information with another person involved in your supports (such as a parent, employer, or health provider) 

  • You would reasonably expect information to be shared with another professional for a purpose directly related to providing your services 

To support high-quality services, your career counsellor may discuss session matters with a supervising rehabilitation counsellor or professional mentor. These discussions are for professional supervision and remain confidential. 

We keep brief and relevant records to support your care.

All personal information is managed in line with our Privacy Policy. 

Confidentiality & Limitations 

Your information is kept confidential and used only to provide vocational services. There are limited situations where information may need to be shared, including if there is: 

  •  a serious risk of harm,  

  • concerns for a child or vulnerable person,  

  • or where required by law.  

Your counsellor may consult with supervisors or colleagues for professional support. These discussions remain confidential. 

By signing this form, you consent to Reframe Careers sharing relevant information with your case manager, LTC Co-ordinator and therapy team to support your goals. You can withdraw this consent at any time or let us know if there are people you do not want us to contact. 

We keep brief and relevant records to support your care.
All personal information is managed in line with our

Copy of Client's NDIS Plan

It is essential for Reframe Careers to have a copy of the My Profile and Goals sections of the NDIS Plan.  This ensures we are working towards your stated Goals, which are required for completing reports for the NDIS.

Schedule of Supports/Services

The NDIS has introduced funding periods and Plan components.

This Schedule of Supports outlines the total plan duration along with the funding allocated to each funding period.

Please note the following:

  • If there are any unused support hours at the end of a funding period, Reframe Careers will automatically roll these over to the next funding period to support continuity of service delivery.

  • If the allocated hours for a funding period are exhausted, services will be paused until the commencement of the next funding period unless we are notified of additional funding being made available within the current funding period.

  • If additional funding is required beyond the agreed total in the current Schedule of Supports, Reframe Careers will communicate this with the Client and provide an amended/updated Service Agreement for their approval. This will also reflect any increase in funding within the funding periods.

ALL funding periods will be below, with the allocation of dollars for Reframe Careers and type of service required.

If there are multiple services required, there will be a new line for each service, so that the dollar allocation is accurate.

If the NDIS Plan is the old lump sum format, the total dollar allocation and type of service required  will be below.

Based on the information you have provided, we believe the supports listed are reasonable and necessary for your needs. 

Please tell us if your information changes or is incorrect. If we do not have up-to-date information, the supports recommended may not be suitable. 

Where possible, we will provide at least 24 hours’ notice if an appointment needs to be changed.

We do not manage your NDIS funding or budget. Please check your available funding before services begin. 

Scope of Services

Support to work towards employment-related NDIS Goals via one or more of the following:
Finding and Keeping A Job

  • Employment Related Assessment, Counselling and Advice: Guidance in identifying personal barriers to employment, setting work goals, and developing a tailored plan.

  • Employment Assistance: Support to build skills for work, find and keep a job.

Description of Scope:

  • Assessment, support planning, and delivery of services to prepare for, secure, or retain employment in the open or supported labour markets (including self-employment and micro-enterprises).

  • Assistance in accessing and sustaining education opportunities.

  • Disability-related support for transitioning into new educational or employment settings.

Delivery Mode and Travel:

  • In Person or Online: Billed as time taken (sessions are typically one hour).

  • Non-Face-to-Face Support: Includes time spent onboarding (including reviewing background reports), report writing, documentation, and communication with Participant, Allied Health Professionals, Support Coordinators, Plan Managers, and other relevant parties.

  • Travel Time: Up to 30 minutes one way at the hourly rate specified in the table below. Return travel (max 30 mins each way) is charged if there are no other appointments for Reframe Careers on that day.

  • Non-labour Provider Travel: $1.00 per kilometre travelled to the Client. Return travel km is charged if there are no other appointments for Reframe Careers on that day.

Support to work towards employment-related NDIS Goals via one or more of the following:
Improved Daily Living

  • Therapeutic Supports: Capacity Building Supports provided by a Rehabilitation Counsellor (member of Australian Society of Rehabilitation Counsellor).

Description of Scope:

  • Assessment, support planning, and delivery of services to prepare for, secure, or retain employment in the open or supported labour markets (including self-employment and micro-enterprises).

  • Assistance in accessing and sustaining education opportunities.

  • Disability-related support for transitioning into new educational or employment settings.

  • Assistance with finding, maintaining or keeping a job.

Service Fees

Fees are charged in line with widely accepted service standards for Career Counselling services.

All rates include GST.

Services provided by the Principal Rehabilitation Consultant, Jane Allen, are charged at $252.00 per hour.

All other Professionals working with Reframe Careers are charged at $242.00 per hour.


Conditions:

  • Payment will only be charged once actual service has been delivered to you

  • If you need to cancel or reschedule, please let us know as early as possible

  • Please review the Cancellation clause of this Agreement

Fees are charged in line with the NDIS Price Guide. If your funding changes, runs low, or ends, we will contact you to discuss next steps before continuing services where possible. 

Conditions:

  • Reframe Careers will increase fees in line with changes to benchmark pricing within the NDIS Price Guide

  • Payment will only be charged once actual service has been delivered to you

  • If you need to cancel or reschedule, please let us know as early as possible

  • Please review the Cancellation clause of this Agreement

Reframe Careers will seek payment for their provision of services after the services have been delivered in accordance with the Client's NDIS Plan funding type noted below:

Payment is due by Electronic Funds Transfer within 14 days of invoicing. 

Rights & Responsibilities

Your Rights

You have the right to:

  • Be treated with dignity and respect

  • Exercise choice and control over your supports

  • Be involved in decisions that affect you

  • Receive clear, accessible information

  • Give informed consent and withdraw it at any time

  • Have your privacy and personal information protected

  • Provide feedback or make a complaint without disadvantage

  • Access interpreters, communication supports, and advocacy if needed

Our Responsibilities

We will:

  • Provide supports suited to your needs, goals, and preferences

  • Support your right to choice and decision-making

  • Ensure workers are appropriately skilled and trained

  • Deliver services in line with the NDIS Practice Standards, NDIS Act and Rules, and Australian Consumer Law

  • Obtain informed consent before supports begin

  • Review supports with you regularly

  • Communicate openly, honestly, and respectfully

  • Protect your privacy and securely manage your information

  • Seek your consent if students are involved in your supports


Your Responsibilities

You agree to:

  • Tell us what supports you want and advise of any changes

  • Treat workers with dignity and respect

  • Notify us of changes to your Service Agreement or NDIS Plan

  • Give at least 24 hours’ notice to cancel a service

  • Give 2 business days’ notice to end services (fees apply up to that date)

  • Not record supports without consent

  • Pay invoices in line with agreed terms (if Self-Managed)

Your rights as a Client

As a Client, you have the following rights, which include:

  • The right to refuse any particular service

  • The right to terminate services at any time

  • The right to ask questions, access information and be informed about services, and

  • The right to ask to be referred to a different team member if you feel we are not right for each other.

To help keep you, our workers, and others safe, you agree to: 

  • Work with Reframe Careers at the start of services to identify any risks and support reasonable actions to reduce risk and promote positive outcomes 

  • If you have NDIS Behaviour Support funding, use this funding when requested to help reduce safety risks to you or others 

If services are provided in your home, you agree to: 

  • Allow Reframe Careers to complete a home safety risk assessment before services begin to meet Workplace Health and Safety requirements, if requested or required

  • Inform us of any access requirements (e.g. preferred entry) 

  • Take reasonable steps to maintain a safe environment, including not smoking, securing pets, and not being under the influence of non-prescription drugs or excessive alcohol during services 

If you do not respond at a scheduled visit, you consent to Reframe Careers taking reasonable steps to confirm your safety. This may include contacting your nominee or decision-maker, or notifying emergency services if required. 

Your Safety

Intellectual Property

All materials, tools, resources, reports, templates, and content created or used by Reframe Careers in providing services remain the intellectual property of Reframe Careers. This includes anything developed, adapted, or modified as part of this Agreement. 

Limitation of Liability

We do our best to provide safe, professional services. If something goes wrong, our responsibility is limited to the amount you have paid us for the service involved. 

We are not responsible for things outside our control, actions taken by you or others, or for services not achieving a specific outcome. We are also not responsible for indirect losses, such as lost income or opportunities. 

You keep all rights you have under the Australian Consumer Law that cannot legally be taken away. 

Limitation of Liability 

To the maximum extent permitted by law: 

  • You agree that you have not relied on any statement, promise, representation, or information provided by Reframe Careers that is not expressly set out in this document. 

  • This document sets out the full terms relating to the services. Any conditions or warranties that may otherwise be implied by law are excluded, except where they cannot legally be excluded. 

  • Reframe Careers’ total liability arising from or in connection with this document is limited to the amount you have paid to Reframe Careers for the services to which the claim relates. 

  • Reframe Careers is not liable for any loss or damage arising from: 

    • Events or circumstances beyond our reasonable control 

    • Acts or omissions of you, your family members, guardians, nominees, or representatives 

    • The services not achieving an expected outcome, benefit, or result 

    • Any indirect or consequential loss, including loss of income, business, opportunity, goodwill, reputation, savings, or data 

  • You agree to indemnify Reframe Careers for any loss, damage, or liability arising from a wrongful act or omission by you, your guardians, nominees, or representatives. 

Nothing in this document excludes, restricts, or modifies any consumer guarantee, right, or remedy available to you under the Australian Consumer Law that cannot lawfully be excluded or limited. 

Cancellation Policy

  • If you need to cancel or reschedule, please let us know as early as possible

  • Cancellation fee applies if there is less than 24 hours notice for a booked appointment.

  • Late cancellations may incur a fee in line with NDIS Rules and our Agreement, however we aim to discuss this with you openly and fairly

  • We will try to give you 24 hours notice if we need to cancel an appointment on our end for any reason, and communicate this with you

  • We may cease services if there are repeated unexplained cancellations and we can't work this through with you and/or your nominee

  • The cancellation fee is the duration of time that the appointment was scheduled for, as well as any travel undertaken, unless we are able to fill this time with non face to face services for you.

We understand that plans can change and appointments may sometimes need to be cancelled or rescheduled. Where possible, we ask that you provide at least 24 hours’ notice for any cancellation. 

If less than 24 hours’ notice is given, a 50% cancellation fee will apply. 

We will try to give you 24 hours notice if we need to cancel an appointment on our end for any reason, and communicate this with you.

We understand that plans can change and appointments may sometimes need to be cancelled or rescheduled. Where possible, we ask that you provide at least 24 hours’ notice for any cancellation.

We may cease services if there are repeated unexplained cancellations and we can't work this through with you.

Refunds 

Reframe Careers maintains professional standards and makes every effort to deliver high-quality services. For this reason, refunds are not offered for services provided. 

Outstanding Fees

Outstanding fees need to be paid prior to further services being provided.  If fees are not paid, Reframe Careers may contact the following (where applicable) to recover the outstanding debt:

  • Plan Manager

  • Support Coordinator

  • LAC/ECEI Partner

  • NDIA

  • Debt collector

  • The Client / Client's Representative will be liable for any outstanding debts that are not payable by the NDIA and also the costs associated in recovering such debts.

Goods & Services Tax (GST)

For the purposes of GST legislation, the Parties confirm that: 

  • A supply of supports under this Service Agreement is a supply of one or more of the reasonable and necessary supports specified in the statement included, under subsection 33 (2) of the National Disability Insurance Scheme Act (NDIS Act), in the Client's NDIS plan currently in effect under section 37 of the NDIS Act;

  • the Client's NDIS plan is expected to remain in effect during the period the supports are provided; and

  • the Client / Client's Authorised Representatice will immediately notify Reframe Careers if the Client's NDIS plan is replaced by a new plan, or if the Client stops being a client within the NDIS.

Consent to use Documentation Aides: Everbility

Reframe Careers uses a secure documentation tool called Everbility to help accurately record session notes and outcomes. This allows us to focus more on our conversation with you, rather than manual note-taking, and supports efficient report writing and documentation. 

Your informed consent is required for us to use this technology as part of your services. 

All personal information collected through Everbility is handled with strict confidentiality and in line with the Privacy Act 1988 and the Privacy Amendment (Enhancing Privacy Protection) Act 2012. The use of Everbility is solely to support service delivery and improve your experience. 

Privacy and Data Protection 

  • Everbility complies with the Australian Privacy Principles, the Privacy Act, HIPAA, and GDPR 

  • Information is used only for the purpose of providing your services through Reframe Careers 

  • Notes and reports are encrypted both in transit and at rest 

  • Data provided to Everbility is not used to train AI models and is not stored on OpenAI servers 

  • Your information is not visible or accessible to other Everbility users 

  • Everbility’s full Privacy Policy is available at: www.everbility.com/privacy 

  • Everbility’s Trust Centre is available at: www.everbility.com/trust 

You may withdraw your consent to the use of this technology at any time by notifying Reframe Careers.

Changes to this Service Agreement

If changes to your supports are needed, we will discuss and review this Service Agreement together.
Changes require at least 
two weeks’ notice.
 

Changes to your support needs may affect fees and funding. This may require a review of your supports, available funding, or your NDIS Plan, and a quote may be provided if required. 

You agree to tell Reframe Careers immediately if your funding is suspended or replaced.

Any agreed changes will be updated in the Schedule of Supports and confirmed in writing. 

Reframe Careers may ask to review this Agreement if services are regularly changed, cancelled, or not attended. 

Ending this Service Agreement

Services may be placed on hold if payment has not been received or claimed within 14 days of service delivery. 

If this Agreement needs to end, we will provide fair and reasonable notice where possible.

The Agreement will automatically end if Reframe Careers can no longer deliver services under your funding, or if you are no longer eligible for that funding.
 

In cases of a serious breach, services may end immediately. 

If a concern or disagreement arises, we will first try to resolve it informally. If this is not successful, our complaints process will be followed, with your rights and wellbeing always prioritised.
